Capillary leak syndrome: State of the art in 2021

Capillary leak syndrome (CLS) is an increasingly acknowledged multifaceted and potentially lethal disease. Initial nonspecific symptoms are followed by the intriguing CLS hallmark: double paradox associating diffuse severe edema hypovolemia, along with hemoconcentration hypoalbuminemia. Spontaneous resolutive phase often associated poor outcome due to iatrogenic fluid overload during phase. mainly triggered drugs (anti-tumoral therapies), malignancy, infections (mostly viruses) inflammatory diseases. Its idiopathic form named after its eponymous finder: Clarkson's pathophysiology involves a severe, transient multifactorial endothelial disruption which mechanisms still unclear. Empirical based-on-experience treatment implies symptomatic care acute (with eventual addition of amplifying cAMP levels in severest cases), prophylactic use monthly polyvalent immunoglobulins prevent relapses. As literature scattered, we aimed collect summarize current knowledge on facilitate diagnosis, understanding management.
